We would like to ask you to provide us with information about yourself for the purposes of enabling us to monitor diversity within our workplace.  We will use any information that you provide to us in order to identify and keep under review the equality of opportunity or treatment between groups of people within our workplace, in order to help us make sure that such equality is promoted and maintained.  The information will not be used for any other purpose and, in particular, it will not be used to make any decisions about you or any other individual.

You do not have to give us this information if you do not want to (there will be no adverse consequences for you if you choose not to do so).  If you choose to give us this information, you can opt out of our processing the information at any time by giving us notice, in which case we will stop processing the information and will delete it.

If you choose to give us the information, then unless you opt out of us processing it (see above), we will continue to process it while you continue to be employed by us and for six years after your employment ends, after which we will delete it.
